Monarch Healthcare Management is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our Float Pool across Minnesota communities. This role offers the flexibility to pick up shifts based on facility needs and provides exposure to multiple skilled nursing and assisted living settings.

Qualifications

• Graduate of a nursing education program approved by the Minnesota Board of Nursing.
• Current Minnesota LPN license in good standing.
• Current CPR certification or willingness to obtain.
• Ability and willingness to travel to Monarch communities.
• Strong communication, organization, and teamwork skills.
• Experience in skilled nursing and/or assisted living preferred.
• Passion for providing compassionate, resident-centered care.

Responsabilités

• Provide direct nursing care to residents in skilled nursing and assisted living.
• Participate in the development and modification of resident care plans.
• Implement nursing interventions as delegated, ordered, or prescribed by the RN or physician.
• Administer medications and treatments according to physician orders.
• Monitor resident conditions and reporting changes to the RN and healthcare team.
• Maintain accurate and timely documentation.
• Support resident rights, dignity, and independence.
• Deliver care in accordance with Minnesota Board of Nursing standards, the Resident Bill of Rights, and facility policies.
• Contribute to quality improvement initiatives and resident-centered care practices.
