Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations, engaging with patients in their own homes and bridging gaps in care. The role is primarily traveling across a defined coverage area, with overnight stays possible.

The visit lasts about 45 minutes and includes history review, physical evaluation, and coordination with health plans for additional tests. MD/DO license required; multi-state licensure may be needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area, as required.
