Job summary

Continuing Life in California seeks a part-time Clinical Coach to travel across the state 16-20 hours per week, guiding clinical staff and ensuring high-quality patient care in SNF/RCFE settings.

The role requires deep MDS expertise; AANAC certification preferred. Travel up to 75% within California, with base options in San Diego, Orange County, SF Valley/Ventura, or Pleasanton. Leadership and regulatory know-how are essential.

Qualifications

• Minimum of five years as a Director of Nursing (DON) in a skilled nursing facility.
• BSN or MSN preferred.
• Working knowledge of State and Federal regulations and the survey process.
• RCFE license preferred.
• Expert level knowledge of the MDS and RAI process.
• Current driver's license and access to a personal car; team oriented and flexible.
• Travel up to 75% within California, mainly Southern California.
• Excellent customer service and communication skills.
• Strong organizational and time management abilities.
• Proficiency with technology including Microsoft Office and Point Click Care.

Responsibilities

• Maintains up-to-date knowledge of Federal and State regulations for resident care.
• Makes staff development recommendations and supports clinical protocol updates.
• Audits nursing records and ensures proper record retention.
• Assists with nurse management applicant screening and hiring processes.
• Supports survey readiness with plan of correction and action plans.
• Protects resident and staff confidentiality; promotes dignified care.
