Job summary

Continuing Life LLC in California is seeking a part-time Clinical Coach (16 hours/week) to travel across the state, mentoring clinical staff and ensuring patient care quality.

The role requires deep MDS expertise and an AANAC certification, with base options across San Diego, Orange County, San Fernando Valley/Ventura, or Pleasanton. A DON background is essential for this leadership position.

Qualifications

• Minimum of five years as a Director of Nursing (DON) in a skilled nursing facility.
• AANAC certification is required and must be current.
• MDS knowledge and RCFE experience are essential.
• Must have a valid driver's license and access to a personal car.

Responsibilities

• Maintain up-to-date knowledge of federal and state regulations for SNF and RCFE care.
• Collaborate with leadership to minimize risk through problem-solving.
• Support staff development and implement clinical protocols and policies.
• Audit nursing records and ensure proper retention.
• Assist in nurse management staff screening and recruitment.
• Stay current with clinical trends in senior living.
• Support survey preparation and corrective action planning.
• Maintain confidentiality of resident and employee information.
• Lead EMR system implementation and auditing (Point Click Care).
• Serve as interim DON during vacancies or extended leaves.
• Protect privacy and ensure safety and quality of care.
