Job Summary
Registered Nurse (Home Health) – provide compassionate and individualized care in patients’ homes.
Key Responsibilities
- Assess and monitor patients' health conditions in their home environment.
- Develop and implement individualized care plans.
- Educate patients and families on health management and self‑care.
- Collaborate with a multidisciplinary team to ensure optimal patient outcomes.
Primary Job Duties
- Assesses, interprets, plans, implements, and evaluates patients according to age and diagnosis.
- Manages and coordinates a caseload of patients with a multidisciplinary team.
- Communicates information in accordance with agency policies and discipline guidelines.
- Contributes to program effectiveness.
- Organizes and performs work efficiently.
- Adjusts schedule to enhance agency performance.
- Demonstrates daily commitment to agency values.
- Maintains positive interpersonal relations with all agency members.
- Promotes customer satisfaction.
- Demonstrates mission, vision, and values of the agency daily.
Job Specifications
- Education: Graduated from an accredited nursing program (AD, Diploma, or BSN).
- Licensure / Certifications: Current RN license in NC or SC and CPR certification.
- Experience: Minimum one year RN experience and two or more years of total clinical experience; home health experience preferred.
- Essential Technical / Motor Skills: Hand‑eye coordination for injections, computer use, patient transfer, and care equipment manipulation.
- Interpersonal Skills: Positive interaction with patients, families, physicians, and staff.
- Essential Physical Requirements: Transfer and maneuver objects up to 50 pounds; frequent lifting, pushing, stair climbing, and fine‑motor manipulation.
- Essential Mental Abilities: Assess, plan, evaluate, teach, prioritize duties, and learn new techniques.
- Essential Sensory Requirements: Visual assessment for dressing changes and IV management; hearing for auscultation, alarms, and communication.
- Exposure to Hazards: Noise, blood‑borne pathogens, blood spills, needle punctures, animals, traffic hazards, weather extremes, and potential safety threats during home visits.
- Hours of Work: Variable Monday‑Friday, weekends, holidays; flexible schedule to accommodate staffing needs.
- Population Served: Adolescents, adults, geriatrics, and pediatrics.
- Driver Requirements: Valid North Carolina driver’s license and operational vehicle.
