Job summary

On Site Medical in Oklahoma City is seeking a Nurse Practitioner to deliver comprehensive wound care across assigned facilities. This mobile role travels to skilled nursing and long-term care sites to assess, diagnose, and manage acute and chronic wounds in collaboration with the care team.

You will work independently with strong clinical judgment, document encounters in the EMR, educate patients and staff, and coordinate referrals as needed.

Qualifications

• Current, unrestricted NP license in Oklahoma.
• National NP certification (FNP/AGNP/AGACNP or equivalent).
• Current prescriptive authority as applicable.
• Strong clinical assessment and decision-making skills.
• Ability to work independently in a mobile healthcare environment.
• Valid driver’s license.

Responsibilities

• Perform comprehensive wound assessments and document findings.
• Develop and implement evidence-based wound care plans.
• Diagnose and manage acute, chronic, and nonhealing wounds.
• Monitor healing progress and adjust treatment as needed.
• Travel to skilled nursing and long-term care facilities for care.
• Educate patients, families, and facility staff on wound care and prevention.
• Coordinate referrals to specialists when necessary.
• Maintain EMR documentation with accuracy and completeness.
