• Develop and implement individualized treatment plans with measurable goals; provide therapy focused on communication, cognition, and swallowing interventions.
• Monitor resident progress, document outcomes, and update treatment plans as needed.
• Collaborate with an interdisciplinary team (nursing, therapy, social work) to coordinate care and optimize resident outcomes.
• Educate residents, families, and care partners on communication strategies and support needs.
• Ensure documentation and practice comply with regulatory and facility policies, including HIPAA.
