Critical Connection, Inc. is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a job in Gainesville, Virginia. This role focuses on patient care and collaboration within a quality-driven SNF, suitable for new graduates with mentorship available.

Qualifications

• Active (or pending) state PT license.
• SNF experience preferred but not required.
• Strong clinical reasoning and communication skills.

Responsibilities

• Evaluate and treat patients recovering from illness, injury, or surgery.
• Develop and update individualized plans of care (POC).
• Provide 1:1 skilled therapy to primarily Medicare patients.
• Collaborate with nursing, OT, SLP, and rehab leadership.
• Document evaluations, progress notes, and discharge summaries.
• Educate patients and families on mobility, safety, and recovery.
