Job summary

Critical Connection, Inc. is looking for a travel Skilled Nursing Facility (SNF) Physical Therapist in Hilliard, Florida. The ideal candidate will join a quality-driven SNF focused on restoring patient mobility, independence, and quality of life.

Responsibilities include evaluating and treating patients, developing care plans, and collaborating with an interdisciplinary team. New graduate PTs are welcome, with mentorship offered. Competitive weekly pay, first-day medical benefits, and a $1,200 CEU allowance are provided.

Qualifications

• Active or pending state PT license.
• SNF experience preferred, not required.

Responsibilities

• Evaluate and treat patients recovering from illness, injury, or surgery.
• Develop and update individualized plans of care.
• Provide 1:1 skilled therapy to primarily Medicare patients.
• Collaborate with nursing, OT, SLP, and rehab leadership.
• Document evaluations, progress notes, and discharge summaries.
• Educate patients and families on mobility, safety, and recovery.
