Qualifications

• SNF experience preferred but not required.

Responsibilities

• Evaluate and treat patients recovering from illness or injury.
• Develop and update individualized plans of care.
• Provide 1:1 skilled therapy primarily to Medicare patients.
• Collaborate with nursing and rehabilitation leadership.
• Document evaluations and progress notes.
• Educate patients and families on recovery.
