Job summary

Jackson Therapy Partners is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week position in Dousman, Wisconsin. This role requires an accredited Physical Therapy degree and an active Wisconsin state license.

You will assist patients in recovering from injuries and managing chronic conditions in a skilled nursing environment. Benefits include full medical options, 401(k) matching, and housing options when eligible.

Qualifications

• Active Wisconsin state license or compact license required.
• Experience in skilled nursing, assisted living, or memory care settings required.
• Willingness to work M–F days and one weekend per month.

Responsibilities

• Help patients recover from injuries and manage chronic conditions.
• Provide patient-centered rehabilitation.
• Participate in interdisciplinary care teams.

Skills

Experience working in skilled nursing facility

Experience managing mixed caseloads

BLS certification

Education

Bachelor's, Master's, or Doctoral degree in Physical Therapy
