Job summary

Jackson Therapy Partners is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week position in Dousman, Wisconsin. This role requires an accredited Physical Therapy degree and an active Wisconsin state license.

Jackson Therapy Partners is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a position in Dousman, Wisconsin.

You will help patients recover from injuries, manage chronic conditions, provide rehabilitation, reduce fall risk, and participate in interdisciplinary care teams to optimize outcomes for older adults.

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Wisconsin state license or compact license recognized for practice prior to the start of the assignment.
• BLS certification from AHA or ARC may be required.
• Experience working in skilled nursing facility, assisted living, independent living, or memory care settings is required. Variety of caseloads expected.
• Experience using NetHealth (electronic medical record) is preferred.
• Willingness to work M–F days and one weekend per month as part of the schedule.

Preferred Qualifications

• Previous travel therapy experience preferred.
• Experience managing mixed caseloads including high‑acuity and geriatric patients preferred.
