Job summary

Amergis is seeking a School Speech-Language Pathologist (SLP) for a travel assignment in Tolleson, AZ. The SLP will evaluate students and provide therapy services per individualized plans and physician orders.

Requirements include active SLP licensure in the assignment state; CCC-SLP preferred; CPR if applicable; health certificate; and compliance with professional standards. Must be 18+ and able to meet contract requirements.

Qualifications

• Active SLP licensure in the state of assignment required.
• CCC-SLP preferred and may be required by state/contract.
• Complies with all relevant professional standards of practice.
• Current CPR if applicable.
• TB questionnaire, PPD or chest x-ray if applicable.
• Current Health certificate per contract or state regulation.

Responsibilities

• Perform student evaluations and document therapy services.
• Provide and document therapy per plan of care and physician orders.
• Maintain accurate progress notes and eligibility documentation.
