Sweetwater Care in California seeks an experienced Registered Nurse (RN) to join our ICF team supporting individuals with disabilities. The RN will provide skilled nursing, oversee care plans, and collaborate with a multidisciplinary team to ensure residents’ well-being.

The role requires a CA RN license, CPR certification, and a compassionate, team-oriented mindset; responsibilities include medication administration, record-keeping, and ongoing professional development.

Formación

• Valid Registered Nurse (RN) license in the state of California
• One (1) year experience as an RN; required
• CPR certification
• Strong communication and interpersonal skills
• A genuine passion for providing quality care and support to individuals with disabilities
• Ability to work effectively in a team-oriented environment
• Willingness to adapt to the needs of the residents and facility

Responsabilidades

Facility Information:

• Our facilities are Intermediate Care Facilities for individuals with disabilities (ICF/DD-H and ICF/DD-N)
• Caregiver (DSP) to people supported ratio is usually 1:3, sometimes as high as 1:1 support
• Nursing caseload typically ranges from 6-15 residents
• Our home sizes are small, including 6-bed and 15-bed facilities
• High-acuity care is limited or prohibited by facility regulations
• Our Immediate Care Facilities provide wound care per licensing requirements, ranging from simple wounds to a maximum of stage I pressure ulcers
• Hospice care is very limited and is provided collaboratively with professional hospice care providers who come to our homes

Registered Nurse (RN) Essential Responsibilities:
