Responsibilities

• Assesses patient needs and uses the nursing process to plan, implement, and evaluate care.
• Collects, analyzes, and documents data and nursing diagnoses.
• Documents care plan with nursing diagnoses, outcomes, and interventions.
• Initiates and evaluates patient and/or family education.
• Revises and documents the plan of care as needed.
