Job summary
AMN Healthcare Revenue Cycle in Montrose, CO seeks a Travel Nurse RN Case Manager, Hospice for a 13-week assignment. You will work 40 hours per week, Monday–Friday, 8:00 am–5:00 pm with on-call rotation.
The RN Hospice Case Manager coordinates end-of-life nursing care across private homes, nursing facilities, and assisted living, guiding families and collaborating with interdisciplinary teams to ensure comfort and dignity.
Qualifications
- CO/compact RN license
- BLS
- 2 years recent case management experience
- 2 years Hospice experience
Responsibilities
- Coordinates compassionate end-of-life nursing care across private homes, nursing homes, group homes, and assisted living facilities by assessing patient needs, guiding families, and collaborating with interdisciplinary teams to ensure comfort and dignity.
