Job summary

Longevity Health is seeking an RN Field Coordinator to blend hands-on patient care with administrative coordination in long-term care settings. The role involves traveling to nursing homes to support Advanced Practice Providers, prescreening enrollees in EMRs, and coordinating care transitions across facilities under the guidance of the Market Director of Clinical Services.

Qualifications

Must be a licensed registered nurse in good standing.
Associate degree in nursing required; BSN preferred.
Valid state driver’s license with a good driving record and proof of automobile insurance.
Willingness to travel to nursing homes.
Minimum 2 years RN experience.
Excellent verbal and written communication; computer skills.

Responsibilities

Visit multiple nursing homes to support APNs with clinical and administrative tasks.
Prescreen enrollees in EMRs and identify continuity of care needs.
Coordinate with clinical and operations teams on member appointments and transfers.
Assist with prior authorization requests and access to services outside facilities.
Educate facility staff on Longevity Health Plan model of care.
Lead orientation sessions for staff at new facilities and onboarded sites.
