Job summary

Longevity Health is seeking a registered nurse to join our Field Coordinator team. This role travels to nursing homes to support Advanced Practice Providers with clinical and administrative tasks, ensuring smooth care transitions and staff education on our care model.

The ideal candidate has an RN license in good standing, 2+ years of RN experience, and the ability to travel to facilities as requested by the Market Director of Clinical Services.

Qualifications

Licensed RN in good standing.
2+ years of RN experience.
Willingness to travel to multiple nursing homes.

Responsibilities

Visit nursing homes to support clinicians with clinical tasks.
Prescreen enrollees in EMRs and assess care needs.
Coordinate with clinical and operations teams on appointments.
