Responsibilities

• Manage inpatient case load (14-20 patients) in a hospital setting.
• Coordinate with hospitalists, case management staff and leadership.
• Utilize EPIC EMR and Careport for referrals and documentation.
• Maintain and monitor patient progression and discharge planning.
