Job summary

TotalMed RN is seeking a travel nurse RN Case Management for a travel nursing job in La Mesa, California. This assignment runs for 13 weeks with a 40-hour work week, day shift. The ideal candidate has 1–2 years of recent case management experience, a current RN license, and BLS certification.

Strong communication and teamwork are essential to advocate for patients and coordinate care across disciplines. You will evaluate new patients, create and update comprehensive short- and long-term care

Qualifications

• 1-2 years of recent case management experience required.
• Current RN license required; BLS preferred.
• Related certifications may be required before beginning assignment.

Responsibilities

• Evaluate and assess new patients.
• Create and update comprehensive short- and long-term care plans.
• Educate families and patients about resources and options.
• Collaborate with other RNs, doctors and social workers.
• Document and submit medical records.
• Interface with insurers to support appropriate decisions and care.
