Job summary

FreedomCare is seeking a Case Manager, RN to join our Ohio team and conduct in-home assessments, orientations for patients and caregivers, and ongoing care management. This field-based role requires frequent travel to patient homes in Akron, OH, and offers opportunities to coordinate care with PCAs and interdisciplinary teams.

Qualifications

• Valid and active RN license issued by the Ohio Board of Nursing.
• Ability to travel within assigned geographic area (usually by car).
• Valid driver’s license and car insurance required.
• Proficient with Case Management Systems and MS Office.
• Strong customer service with empathy, active listening and patience.
• Bilingual is a plus.

Responsibilities

• Conduct initial in-home clinical assessments and coordinate service activation.
• Develop, implement, and manage patient activity plans.
• Document changes and maintain accurate client records.
• Coordinate care with physicians and interdisciplinary teams.
• Provide coaching to optimize use of best practices and PCA education.
• Monitor PCA notes and changes in patient condition.
• Ensure regulatory and policy compliance.
• Perform other duties as needed by supervisor.
