Qualifications

• RN with Case Management specialization.
• 1–2 years of recent case management experience.
• ASAP start date for assignment.
• 17-week duration and travel assignment.
• 40 hours per week, 8-hour days.

Responsibilities

• Evaluate and assess new patients and develop care plans.
• Create and update comprehensive short- and long-term patient care plans.
• Educate families and patients about resources and options.
• Collaborate with RNs, physicians, and social workers.
• Document and submit medical records accurately.
• Communicate with Insurance companies to coordinate or appeal decisions.
• Analyze results and trends to improve cost-effective outcomes.
