Job summary

TotalMed RN is seeking a travel nurse RN Case Management for a travel nursing job in Carmichael, California.

As a Case Manager Travel Nurse, you will evaluate patients, develop care plans, educate families, and coordinate with clinicians and insurers to optimize care and reduce readmissions. This 17-week travel assignment requires 40 hours per week, 8-hour day shifts, and an ASAP start.

Qualifications

• 1-2 years of recent case management experience.
• BLS certification required.
• Travel RN case manager position with ASAP start.

Responsibilities

• Evaluate and assess new patients.
• Create and update comprehensive short- and long-term care plans.
• Educate families and patients about resources and options.
• Collaborate with other RNs, doctors and social workers.
• Document and submit medical records.
• Interact with insurance to advocate or appeal decisions.
• Analyze results to improve financial outcomes and reduce costs.
