Responsibilities

• Manage a caseload of patients and ensure care that supports clinical and financial outcomes.
• Coordinate care prescribed by an interdisciplinary team using patient assessment, guidelines and payer regulations.
• Utilize protocols and response to therapies to assess illness from pre admission to post discharge.
