Job summary

TotalMed RN seeks a travel nurse RN Case Management for a 13-week assignment in Boston, MA. The RN will handle patient assessment, care planning, and coordination with a multidisciplinary team to optimize outcomes.

Responsibilities include education of families, documentation, and collaboration with insurers to support decisions while managing fluctuating caseloads and maintaining quality patient care in a dynamic travel setting.

Qualifications

• RN case management experience required.
• BLS certification required.
• 1–2 years recent case management experience preferred.

Responsibilities

• Evaluate and assess new patients and needs.
• Create and update comprehensive short- and long-term care plans.
• Educate families and patients about resources and options.
• Collaborate with other RNs, physicians and social workers.
• Document and submit medical records accurately.
