Job summary

Healthforce is seeking a Registered Nurse - Case Management for a travel assignment in Fort Myers, FL. The role focuses on case management and discharge planning within an acute care hospital to support a readmission-reduction initiative.

The assignment runs for 13 weeks starting 12/07/2026, with guaranteed 40 hours per week. Shift is 8-hour days on a standard 8x5 schedule, offering travel support and extended assignment opportunities.

Qualifications

• Florida RN license (FL/Compact) must be on profile.
• BLS certification held.
• 2+ years of case management experience in an acute care setting.
• Degree accreditation on profile or 5 years case management experience.

Responsibilities

• Conduct comprehensive case management assessments to identify discharge needs, risks, and barriers to safe transition.
• Develop and coordinate individualized care plans with interdisciplinary team; arrange post-acute services as appropriate.
• Facilitate timely discharge planning and patient education with clear communication to patients, families, and care teams.
• Monitor patient progress, document utilization and outcomes, and support payer guidelines and hospital policies.
• Collaborate with physicians, social workers, and ancillary staff to reduce readmissions.
• Participate in readmission-reduction initiatives and float within the unit and campuses as needed.
