Job summary

Amergis Healthcare Staffing is seeking a Case Manager Registered Nurse (RN) for travel to Fort Myers, FL. You will coordinate continuum of care, optimize resource use, and ensure adherence to medical regimes across assigned patients.

Qualifications

• Current RN licensure in state practicing.
• At least one year of Case Management experience preferred.
• Complies with all relevant professional standards of practice.
• Current CPR if applicable.
• TB questionnaire, PPD or chest x-ray if applicable.
• Current Health certificate (per contract or state regulation).
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsibilities

• The RN Case Manager is responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with medical regime.
