Job summary

TotalMed RN is seeking a travel nurse RN Case Manager for an Acute Care Case Management assignment in Burbank, CA. The role runs 26 weeks, with 40 hours per week and 8-hour day shifts as travel employment.

The RN Case Manager will evaluate patients, plan care, educate families, and collaborate with RNs, physicians and social workers while documenting records and communicating with insurers to support cost-effective outcomes.

Qualifications

• 1–2 years recent case management experience.
• BLS certification required.
• Must be able to travel to assignment locations.
• Excellent communication and coordination skills.

Responsibilities

• Evaluate and assess new patients.
• Create and update care plans.
• Educate families about options.
• Collaborate with RNs, doctors and social workers.
