HealthForce Healthcare seeks a Registered Nurse – Case Management for a Cape Coral, FL travel assignment. Start 12/07/2026 for 13 weeks with weekly pay range $1,653–$1,689 and guaranteed 36 hours.

Shift: 12-hour evenings in a hospital setting, with potential to float to sister campuses as needed. Responsibilities include comprehensive acute care case management, discharge planning, and collaboration with the multidisciplinary care team to ensure safe transitions and appropriate resource

Formación

• Active RN license required
• BLS for healthcare providers required
• 2+ years of acute care case management preferred
• Willingness to float to sister campuses as needed

Responsabilidades

• Conduct comprehensive case management for acute care patients
• Coordinate discharge planning with patients, families and care teams
• Collaborate with physicians, nurses, and care teams to optimize treatment plans
• Monitor progress and advocate for appropriate services
• Document plans and outcomes in the EHR
• Participate in rounds and care conferences; float as needed
