American Traveler is hiring an experienced RN Care Manager for an acute care hospital setting in North Carolina. The role requires case management experience and an NC or compact RN license, with day shift from 8:00am to 5:00pm and no on-call requirements.

The unit covers Cardiology, Nephrology, Trauma Surgery, Ortho, Vascular, Neurosurgery, GI, Urology, Transplant, and ICU/Stepdown. The assignment includes travel compensation with weekly pay, lodging and meal stipends, and comprehensive

Qualifications

• Active RN license NC or compact required.
• 2+ years of Case Management or Care Management experience in an acute care setting required.
• 2+ years of healthcare experience as a Registered Nurse required.
• Nursing Diploma/ ADN/ ASN from an accredited nursing school required; BSN or MSN preferred.

Responsibilities

• Provide comprehensive assessment, planning, implementation, and evaluation of individual patient care needs.
• Coordinate discharge planning and care progression for patients with complex medical conditions.
• Participate in daily Care Management Touchpoints and CAPP meetings to review patient progression and discharge readiness.
• Consult with Social Work and Utilization Management per departmental protocols.

Requirements

• Strong assessment and critical thinking skills required

Additional Information

• Attend weekly Complex Care Meetings to collaborate on complex patient cases and identify trends
• Consult with Social Work and Utilization Management per departmental protocols
• Coordinate referrals to community resources and post-acute care providers
• Communicate with patients and families regarding medical milestones and discharge plans
• Experience in an educational or teaching hospital setting is beneficial
