Job summary

Prime Time Healthcare Nursing is seeking a travel nurse RN Care Manager for a 26-week assignment in Hyannis, Massachusetts. You will coordinate patient care, utilization review and discharge planning with physicians, patients, families and payers to support safe transitions.

Requirements include an active MA RN license, 3+ years acute care in the past 5 years, recent discharge planning/utilization review, and knowledge of InterQual or equivalent; BSN and case-management certification preferred.

Qualifications

• Active Massachusetts RN registration.
• 3+ years of acute care experience within the past 5 years.
• Recent discharge planning and utilization review experience within the past 4 years.

Responsibilities

• Screen new admissions for discharge planning needs and complete initial assessments.
• Review admission status, level of care and continued stay using InterQual criteria.
• Coordinate discharge plans, referrals and transfers.
• Provide clinical information to payers and help with coverage decisions and appeal rights.
