POSITION SUMMARY

– RN Care Manager

POSITION DUTIES

• – Provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. Enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review and discharge planning. Highly organized professional with great attention to detail, adaptable to frequent change, and compliant with regulatory and departmental guidelines and policies.

MINIMUM REQUIRED QUALIFICATIONS

• – Nursing Diploma or ADN / ASN from an accredited school of NursingNC/compact RN license2 years acute care hospital case management experienceCare manager / discharge planning experience

PREFERRED QUALIFICATIONS

• – EPIC experienceExperience with educational/teaching hospitalBSN or MSN from an accredited school of Nursing
