Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning. Utilization management and revenue integrity.

MINIMUM REQUIRED QUALIFICATIONS

• NH/compact RN license
• BLS
• 2 years case management experience in an acute hospital setting
• Acute hospital experience during the past year with no more than a 90-day break
• Critical care experience
