Location: Madison, WI 53711

Position: Travel Nurse (Full Time, 1.0 FTE, 40 hours/week, Day Shift 8a-5p) with on call (M-F 5p-8a), weekend (F–Mon 5p-8a) rotations.

Overview

Join Agrace, a nationally recognized nonprofit leader in hospice and palliative care. This role involves temporary nursing assignments in emerging hospice markets and existing territories, focusing on patient‑centered care and collaborative interdisciplinary teamwork.

Responsibilities

• Patient Evaluation – Review medical records and collaborate with physicians and IDT to determine hospice eligibility; create HES and document data.
• Patient & Family Assessments – Perform assessments covering physical, pain, psychosocial, emotional, spiritual, environmental, and educational needs; document findings and report changes to IDT.
• Patient Visits – Respond to changes, provide interventions, pain management, education, and adjust care plans as necessary.
• Plan of Care (POC) – Formulate, implement and revise individualized POC, manage visits, delegate to disciplines, oversee CNAs and LPNs.
• Additional Duties – Participate in team meetings, precept new staff and students, attend on‑call rotations, and serve as a consultant for hospital/clinic staff and families about end‑of‑life issues.

Travel Requirements

• Valid driver's license with auto insurance and access to reliable vehicle.
• Must be willing to fulfill travel assignments based on contract and assignment locations.
• Travel experience preferred.

Qualifications

• One‑year nursing experience required.
• Minimum age 18.
• Knowledge of hospice and end‑of‑life processes and ability to maintain professional boundaries.
• Strong communication, collaboration, and flexible time management skills.
• Proficiency in Microsoft Office and electronic health records (Epic preferred).
• Competent with proper body mechanics, safe patient handling, and standard precautions.
