Job summary

Qualifications

• Graduate from an accredited School of Nursing; ADN required, BSN preferred.
• Current license to practice as an RN in New York State and/or Connecticut.
• BLS certification required; specialized certifications as needed.

Responsibilities

• Assess physiologic health status and document findings.
• Assess psychosocial health and plan education for patients and families.
• Identify outcomes and develop an individualized plan of care with priorities.
• Implement the plan of care and administer treatments safely.
• Collaborate with physicians and care team; plan discharge and referrals.
• Educate patients and families to support post-hospital care.
