• Strong communication and documentation skills; proficiency with electronic health records.

Responsibilities

• Provide in-home nursing care to patients within the service area, delivering safe and evidence-based clinical interventions.
• Conduct comprehensive nursing assessments during visits; monitor vitals; administer prescribed medications; perform wound care and symptom management; educate patients and family caregivers.
• Document all care provided and patient status in the electronic health record; communicate changes to the supervising clinician and care team.
• Coordinate with the interdisciplinary team to implement care plans and participate in patient enrollment activities for the home-based program.
• Ensure safety, infection control, and quality standards during every home visit.
