CVS Health is seeking a Field-Based Case Manager RN in Roanoke, VA to lead assessments, care planning, and coordination for LTSS members. The role blends autonomy with collaboration and requires travel up to 75% to support diverse health needs within the community.
Ideal candidates are RNs with Virginia licensure, 3 years of relevant clinical experience, and managed care familiarity; bilingual English/Spanish is a plus. Work-from-home flexibility with weekday hours is provided.
Qualificações
- RN with current unrestricted Virginia licensure.
- 3 years clinical experience (Medical-Surgical / Behavioral Health / Long-Term Care).
- 1+ years managed care experience.
- Willingness to travel up to 75%.
- Proficiency with multiple computer systems.
Responsabilidades
- Conduct in-depth, face-to-face assessments to identify member needs.
- Develop and implement individualized care plans.
- Coordinate medical, behavioral and community-based services.
- Monitor progress and adjust care strategies to improve outcomes.
- Advocate for members to ensure access to high-quality, appropriate care.
