Qualifications

• RN with current Virginia licensure and active RN license in VA.
• 3 years clinical experience in Medical-Surgical, Behavioral Health or LTSS.
• 1+ years managed care experience and coordination of services.
• Proficient with computer systems and multiple databases.
• Willing and able to travel up to 75% of time.

Responsibilities

• Conduct in-depth, face-to-face assessments to identify member needs.
• Develop and implement individualized care plans.
• Coordinate medical, behavioral and community-based services.
• Monitor progress and adjust care strategies to improve outcomes.
• Advocate for access to high-quality, appropriate care.
