Qualifications

• Must have a Master's degree in Social Work.
• CMS Long-Term Care Basic Training and SMQT required.
• Two years of surveyor experience in long-term care facilities.
• Current LCSW licensure in good standing.
• Ability to travel up to 75% of the time.
