Job summary

Sweetwater Care is seeking a dedicated Physical Therapy Assistant to support patients' recovery and rehabilitation, helping them regain functional ability after illness, injury, or surgery. You will work under a Licensed Physical Therapist to implement therapy plans and monitor progress.

Responsibilities include documenting services for billing, following the Plan of Care, and collaborating with the care team to address patient needs.

Qualifications

• High School Diploma required.
• Requires valid Physical Therapy Assistant license or certification in the state of practice.

Responsibilities

• Provide support care of residents aimed at recovery and rehabilitation.
• Assist in quality patient care under supervision of licensed therapists.
• Document all services to ensure accurate and timely billing.
• Follow the Plan of Care and organizational guidelines to meet standards.
• Under supervision of a Physical Therapist, identify and develop care plans.
• Refer to other disciplines as needed under supervision.
• Other duties may be assigned with or without notice.
