Job summary

Critical Connection, Inc. is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist in Globe, Arizona. This position focuses on restoring mobility and quality of life for short-term rehab and long-term care patients.

The ideal candidate will value 1:1 patient care and interdisciplinary collaboration. Strong communication skills and an active PT license are required. Benefits include competitive weekly pay, first-day medical benefits, and a $1,200 CEU allowance.

Qualifications

• Active (or pending) state PT license required.
• SNF experience preferred, but not mandatory.
• New graduate PTs are welcome with mentorship available.

Responsibilities

• Evaluate and treat patients recovering from illness, injury, or surgery.
• Develop and update individualized plans of care (POC).
• Provide 1:1 skilled therapy to primarily Medicare patients.
• Collaborate with nursing, OT, SLP, and rehab leadership.
• Document evaluations, progress notes, and discharge summaries.
• Educate patients and families on mobility, safety, and recovery.
