Job summary

Elite Medical Staffing is seeking a Physical Therapist (PT) for a Skilled Nursing Facility assignment in Globe, AZ. This contract role offers competitive pay of $60/hour and a consistent weekday schedule (M-F, 8am to 5pm).

The PT will assess residents, develop and implement treatment plans to improve mobility and independence, and document progress in the EMR. AZ license + SNF experience required; reliable transportation is essential.

Qualifications

• Active PT license and ability to work in AZ.
• Minimum 1 year SNF experience.
• Reliable transportation and punctual attendance.

Responsibilities

• Utilize EMR to record patients’ medical information and vital signs.
• Evaluate residents’ physical function, strength, balance, mobility, and rehabilitation needs.
• Develop and implement individualized treatment plans to improve mobility and functional independence.
• Provide therapeutic exercises, gait training, balance training, and other skilled physical therapy interventions.
• Monitor resident progress and adjust treatment plans based on clinical findings and goals.
• Educate residents, families, and caregivers on exercises, mobility techniques, and fall prevention strategies.
