Centra Healthcare Allied is seeking a travel Physical Therapist for a 13-week assignment in Eugene, Oregon. This role involves 40 hours per week, days shift, in a SNF/LTC setting, with ASAP start and travel employment.

The candidate should have graduated from an accredited program and hold, or be pursuing, a current state PT license. You will implement care plans, deliver skilled PT services and collaborate across disciplines to achieve patient goals.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist or in process for licensure.

Responsabilités

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services per physician orders.
• Assess, plan, evaluate and modify care to meet goals.
• Collaborate with all disciplines to plan and evaluate team goals.
