Job summary

HealthForce Healthcare in Ukiah, CA is seeking a Physical Therapist for a 13-week assignment in a Short Term Acute Care Hospital, starting 10/05/2026. You will work 40 hours per week on the day shift (8:00 AM–4:30 PM).

The role requires a DPT, active CA licensure, BLS/CPR, EPIC EMR experience, and at least two years of PT practice. Responsibilities include designing and guiding PT treatments, evaluating progress, and coordinating with the care team.

Qualifications

• Doctor of Physical Therapy (DPT) degree; required if licensure obtained after January 2016, or foreign equivalent
• Current Physical Therapy licensure in the state of practice; BLS or CPR certification as required
• At least two years of recent Physical Therapy experience (two years preferred)
• EPIC electronic medical record experience required

Responsibilities

• Plan and conduct segments of physical therapy treatment programs to restore and improve physical function and activity
• Complete evaluations and establish individualized treatment, discharge, and follow-up plans; document progress and communicate with the care team
• Serve as a co-leader in interdisciplinary patient groups; collaborate with other professionals to optimize patient outcomes
• Provide therapy interventions according to established protocols; adjust plans as needed based on patient response
• Supervise assigned assistants and ensure appropriate delegation and supervision of treatment activities
• Maintain accurate, timely clinical documentation and ensure communication of care plans to patients, families, and other care providers
