Responsibilities

• Plan and conduct segments of physical therapy treatment programs to restore and improve physical function
• Co-lead interdisciplinary patient groups and collaborate with the care team to optimize patient outcomes
• Perform comprehensive evaluations, establish individualized treatment plans, and implement evidence-based therapy interventions
• Develop and communicate discharge plans; document clinical progress and outcomes within established timelines
• Supervise assigned therapy assistants, ensuring safe and effective service delivery
