The Judge Group is seeking a skilled Occupational Therapist (OT) to provide in-home evaluations and treatment across Kent and Queen Anne's Counties, Maryland. You will collaborate with RNs, LPNs, PTs, and PTAs to help patients regain function and progress to appropriate levels of care.

Responsibilities include evaluating patients in their homes, completing required assessments, maintaining CTX productivity, and ensuring accurate OASIS documentation.

Qualifications

Occupational Therapist (OT) - Home Health

We are seeking a skilled and compassionate Occupational Therapist (OT) to provide in-home evaluations and treatment to patients throughout Kent and Queen Anne's Counties. The Occupational Therapist will collaborate with an interdisciplinary care team, including Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Physical Therapists (PTs), and Physical Therapist Assistants (PTAs), to help patients improve their functional abilities and progress to the next appropriate level of care. This position is ideal for an experienced home health clinician who is comfortable managing a daily patient caseload, completing required assessments and documentation, and traveling throughout an assigned territory.

32 hours per week | 8:00 AM-4:30 PM

Pay Rate

$50-$55/hr

Key Responsibilities

• Evaluate and treat patients in their homes to support functional improvement, independence, and progression to the next level of care.
• Complete patient evaluations, routine revisits, 30-day assessments, and discharge visits as needed.
• Maintain productivity expectations of 24 points per week, with a daily productivity expectation of approximately 6 points per workday.
• Collaborate with RNs, LPNs, PTs, PTAs, and other members of the interdisciplinary team to coordinate patient care and achieve treatment goals.
• Develop and implement individualized treatment plans based on patient needs and clinical assessments.
• Complete accurate and timely clinical documentation in accordance with home health requirements, including OASIS documentation as applicable.
• Travel daily between patient homes throughout Kent and Queen Anne's Counties.
• Travel to the office as needed to support patient care and operational requirements.
• Communicate effectively with patients, caregivers, and other healthcare professionals to promote continuity of care.

Required Qualifications

• Current and valid Occupational Therapist (OT) license.
• Basic Life Support (BLS) certification.
• Previous skilled home health experience.
• Experience with Homecare Homebase (HCHB).
• Experience completing OASIS assessments and related documentation.
• Ability to travel throughout the assigned service territory.
• Strong clinical judgment, communication, organization, and time-management skills.

Preferred Qualifications

Candidates who meet the required qualifications and bring additional experience in skilled home health, HCHB, and OASIS documentation will be well positioned for success in this role.
