Job summary

Proforma Health Inc. seeks a Travel Home Health Physical Therapy Assistant (PTA) to support patients recovering from injuries under the supervision of a licensed Physical Therapist. Work in home health settings across designated counties.

Requirements include an Associate degree in Physical Therapy, active Oregon PTA license, home health experience, CPR/BLS, and strong communication and patient care skills. Travel between patient locations with mileage reimbursement.

Qualifications

• Associate degree in Physical Therapy from an accredited program.
• Active State PTA license and Oregon PTA license.
• Home health experience required.
• CPR and BLS certifications (may be required).
• Strong communication and patient care skills.

Job description

Role:

Travel Home Health Physical Therapy Assistant (PTA) responsible for supporting patients recovering from injuries or illness under the supervision of a licensed Physical Therapist.

Key Responsibilities

Key Duties

• Deliver therapy services Monday to Friday, 8:00 AM - 4:30 PM.
• Support patient recovery in Multnomah and Clackamas counties.
