Job summary

Proforma Health Inc. seeks a Travel Home Health Physical Therapy Assistant (PTA) to support patients recovering from injuries under the supervision of a licensed Physical Therapist. Work in home health settings across designated counties.

Requirements include an Associate degree in Physical Therapy, active Oregon PTA license, home health experience, CPR/BLS, and strong communication and patient care skills. Travel between patient locations with mileage reimbursement.

Qualifications

• Associate degree in Physical Therapy from an accredited program.
• Active State PTA license and Oregon PTA license.
• Home health experience required.
• CPR and BLS certifications (may be required).
• Strong communication and patient care skills.

Responsibilities

• Provide therapy services to enhance patient mobility, recovery, and well-being.
• Work in home health settings within designated coverage areas.
• Assist with patient assessments and implement treatment plans.
• Travel between patient locations, with mileage reimbursement.
