Job summary
Personal Touch Home Health Care Services Inc. seeks a Travel Skilled Nursing Facility (SNF) Physical Therapy Assistant (PTA) to support patient mobility and recovery. You will work under a licensed Physical Therapist and follow treatment plans to help patients regain movement.
Responsibilities include documenting progress, assisting with transfers, gait training, ADL-focused interventions, and providing therapy in multiple SNFs across New York.
Qualifications
- Associate degree in Physical Therapy from an accredited program.
- Active NY State PTA license.
- BLS certification.
- Preferred experience in skilled nursing or long-term care settings.
Responsibilities
- Assist patients recovering from injuries or illness to improve movement, mobility, and activity levels under the supervision of a licensed Physical Therapist.
- Follow treatment plans, document progress, and support restorative programs, transfers, gait training, wheelchair management, and ADL-focused interventions.
- Provide therapy services in a skilled nursing environment and document patient progress.
