Job summary

Personal Touch Home Health Care Services Inc. seeks a Travel Skilled Nursing Facility (SNF) Physical Therapy Assistant (PTA) to support patients recovering from injuries or illnesses in a SNF setting.

You will work under the supervision of a licensed Physical Therapist to implement care plans and promote mobility, with a focus on adult and geriatric populations. The role requires an accredited Associate degree in Physical Therapy, an active NJ PTA license, and BLS certification may be required.

Qualifications

• Associate degree in Physical Therapy from an accredited program.
• Active New Jersey PTA license.
• BLS certification may be required.
• Experience in skilled nursing or long-term care preferred.

Responsibilities

• Provide patient support during therapy sessions and follow care plans.
• Assist SNF patients to improve mobility and function under a supervising PT.
• Maintain documentation and collaborate with the healthcare team.
